At Integrative Osteopathic Family Medicine of Vermont, you pay one monthly fee for direct primary care services.
For concierge physician services, the patient pays a monthly fee and insurance is charged for any services rendered, leaving the patient responsible for co-pay and co-insurance in addition to their monthly fee.

Direct Primary Care membership is paid in monthly, quarterly or annual installments. Payments can be drafted directly from a checking account or charged to a debit/credit card.
Payment is due at time of visit for any fee-for-service treatments including Integrative Medicine Consultations or Integrative treatments.

Yes. Patients can use money from HSAs and other similar plans to pay for medications, labs, imaging or other healthcare related costs including Integrative treatments like Acupuncture. Sometimes these funds can be used to pay for DPC membership as well. We recommend checking with your HSA plan administrator to see if this is possible.

We are ‘out of network’ and do not bill insurance.
We encourage you to use your flexible spending and health savings account debit cards for our services—these pretax accounts provided by some insurance companies are a great way to offset the cost of your out-of-pocket health care costs.
Please note that you will be responsible for the cost of your visit at time of service.
Your insurance can still cover laboratory testing, imaging, medications, specialty physician referrals or hospitalizations prescribed by our office.

It is not required that patients carry or obtain health insurance to become a patient at Integrative Osteopathic Family Medicine of Vermont. However, we highly recommend patients have some type of catastrophic coverage such as a high deductible insurance with an HSA (Health Savings Account) to cover things like emergency room needs, hospitalization, referrals to specialists and outside testing.

No. We accept patients who have Medicare or Medicaid, but we will not bill either insurance payer. You need only sign a one-time waiver declaring that neither you nor your doctor will directly bill Medicare for our services.
Medicare will still cover any laboratory testing, imaging, medications, specialty physician referrals or hospitalizations prescribed by our office.

Yes. We offer Integrative Medicine Consultations as well as Acupuncture, Osteopathic Manipulative Treatment and other Integrative treatments on a fee-for-service basis. An initial consultation is required prior to receiving Integrative treatments. Click Consultation & Treatments for details.

Yes. We offer Integrative Medicine treatments including Acupuncture, Osteopathic Manipulative treatment (OMT) and other Integrative treatments. One of the benefits of Integrative DPC membership is a discounted rate for a monthly Integrative treatment. Integrative Medicine Versus Functional Medicine
While there are some similarities between Integrative Medicine and Functional Medicine— and “Functional” means different things to different people—we see Functional Medicine as a subspecialty of Integrative Medicine.
At Integrative Osteopathic Family Medicine of Vermont, we focus on a personalized, whole health approach to health care to prevent and reverse chronic illness. As part of our treatment plan we utilize Integrative Family Medicine, Osteopathic Medicine, Acupuncture, Chinese Medicine, Trigger point injections and other modalities to help restore the self healing process.
Functional Medicine, on the other hand, is a model of care which aims to cast a wide diagnostic net with expensive specialty laboratory testing while utilizing multiple supplements as a main aspect of treatment. While this approach has its place, it is not necessary for everyone at all times.
There are clinicians—both conventional physicians (M.D./D.O.) and Naturopathic doctors (N.D.)—in the community who practice Functional Medicine or specialize in Autoimmune conditions, Chronic Lyme, Environmental Toxicity, Heavy Metal Exposure and other conditions.
We often serve as the primary care home for individuals working with clinicians in Functional Medicine or other Integrative Specialty Services.

Once you are a DPC member you can communicate with Dr. Borja when needed. At the same time, Dr. Borja strives to maintain a healthy work-life balance to provide optimal care to you. For routine questions or concerns we ask members to email, text or call anytime during office hours. We will respond in a timely manner that day.
For urgent matters outside office hours, we recommend calling or texting and Dr. Borja will return your communication as soon as possible - usually within a few hours, but at latest within the next business day.
Of course, in case of an emergency, we advise going to your local emergency room or calling 911. We do recommend contacting Dr. Borja if you are hospitalized so he can communicate with your hospital team.
If you are not a member and have a question or concern, we will respond back at our earliest convenience.

Yes, we make house calls and assisted living/nursing home visits. We reserve these visits for DPC members who are unable to leave their homes. We charge $50 per house call within 30 minutes round trip from our office. If the distance is further, please contact us to discuss.
The membership rate for assisted living/nursing home members is $150/month. House call rates apply for medical visits which take place at the assisted living/nursing home facility.

Yes. Buying DPC memberships for employees is tax-deductible and has a superior benefit-cost ratio compared to traditional health insurance plans.

Members may cancel at anytime without penalty. Please keep in mind the following:
Cancellation of membership is effective at the end of the current month and the current month of enrollment is not refundable.
If you pay for membership fees annually, refunds will be prorated for the months not used. For example, if you pay an annual membership in January, you will be billed for January-December. If you decide to end membership in February, you will be refunded for the months of March through December.
You will be charged a re-enrollment fee of $300.00 to reestablish membership unless there are reasonable circumstances for canceling. We reserve the right to determine what is reasonable on a case-by-case basis. The purpose of the re-enrollment fee is to discourage abuse of our services.
