At Integrative Osteopathic Family Medicine, you pay one monthly fee for direct primary care services. For concierge physician services, the patient pays a monthly fee and insurance is charged for any services rendered, leaving the patient responsible for co-pay and co-insurance in addition to their monthly fee.

Direct Primary Care membership is paid in monthly, quarterly, semiannual or annual installments. Payments can be drafted directly from a checking account or charged to a debit/credit card.
Payment is due at time of visit for any fee-for-service appointments including Consultations or Treatments.

Yes. Patients can use money from HSAs and similar accounts to pay for medications, labs, imaging, and other healthcare-related expenses, including integrative treatments like acupuncture. Beginning in 2026, new legislation will explicitly allow HSA funds to be used for DPC memberships as well. We recommend confirming the details with your HSA plan administrator, but this change should make it easier for patients to use HSA dollars to cover DPC care directly.

We are ‘out of network’ and do not bill insurance.
We encourage you to use your flexible spending and health savings account debit cards for our services. These pretax accounts provided by some insurance companies are a great way to offset the cost of your out-of-pocket health care costs.
Please note that you will be responsible for the cost of your visit at time of service.
Your insurance can still cover laboratory testing, imaging, medications, specialty physician referrals or hospitalizations prescribed by our office.

Health insurance is not required to become a patient at Integrative Osteopathic Family Medicine. However, we strongly recommend having catastrophic coverage, such as a high deductible plan with an HSA, to cover emergency care, hospitalization, specialist referrals, and outside testing.
Some patients choose to use a health share program instead of traditional insurance. While health share programs are not insurance, a Direct Primary Care membership may help reduce overall healthcare costs and may be compatible with certain health share arrangements. Patients should review the specific terms of their program.

No. We accept patients who have Medicare, but we will not bill Medicare for DPC membership fees or Integrative treatments. You need only sign a one-time waiver declaring that neither you nor your doctor will directly bill Medicare for our services.
Medicare will still cover any laboratory testing, imaging, medications, specialty physician referrals or hospitalizations prescribed by our office.

Yes. We offer Comprehensive Medical Consultations as well as Acupuncture, Osteopathic Manipulative Treatment and other therapeutic modalities on a fee-for-service basis. An initial consultation is required prior to receiving treatments. Click Consultation & Treatments for details.

Yes. We offer Integrative treatments including Acupuncture, Osteopathic Manipulative treatment (OMT) and other treatments. One of the benefits of Direct Primary Care (DPC) membership is a discounted rate for a monthly Integrative treatment. Integrative Medicine Versus Functional Medicine
Integrative Medicine and Functional Medicine share some common ground, and the term functional can mean different things to different people. We view Functional Medicine as one subspecialty within the broader field of Integrative Medicine.
At Integrative Osteopathic Family Medicine, our focus is on personalized, comprehensive primary care that supports overall health and the body’s inherent capacity for self regulation and healing. Our approach incorporates Integrative Family medicine, Osteopathic medicine, Acupuncture, and other supportive modalities as clinically appropriate.
Functional Medicine typically emphasizes broad diagnostic testing, including specialized laboratory studies, and often relies heavily on supplements as a primary treatment strategy. While this approach may be appropriate in certain clinical situations, it is not necessary or appropriate for every patient or at every stage of care.
Many clinicians in the community, including physicians and naturopathic doctors, specialize in Functional Medicine or focus on complex conditions such as autoimmune disease, chronic infections, environmental exposures, and related concerns. We frequently serve as the primary care home for patients who are also working with Functional Medicine or other integrative specialty clinicians, helping coordinate care and support overall health needs.

Once you are a Direct Primary Care member, you may communicate with Dr. Borja as needed. At the same time, maintaining a healthy work life balance is essential to providing thoughtful, high quality care.
For routine questions or concerns, members are encouraged to email, text, or call during regular office hours. Messages are typically responded to the same business day.
For urgent matters outside of office hours, members may call or text. Dr. Borja will return your communication as soon as possible, usually within a few hours and no later than the next business day.
In the event of a medical emergency, please call 911 or go to the nearest emergency department. If you are hospitalized, we encourage you or a family member to notify Dr. Borja so he can communicate with your hospital care team when appropriate.
For individuals who are not Direct Primary Care members, inquiries will be returned at our earliest availability.

Yes, we make house calls and assisted living/nursing home visits. We reserve these visits for DPC members who are unable to leave their homes. We charge $50 per house call within 30 minutes round trip from our office. If the distance is further, please contact us to discuss.
The membership rate for assisted living/nursing home members is $150/month. House call rates apply for medical visits which take place at the assisted living/nursing home facility.

Yes. Buying DPC memberships for employees is tax-deductible and has a superior benefit-cost ratio compared to traditional health insurance plans.

Members may cancel at anytime without penalty. Please keep in mind the following:
Cancellation of membership is effective at the end of the current month and the current month of enrollment is not refundable.
If you pay for membership fees quarterly, semiannual or annually, refunds will be prorated for the months not used. For example, if you pay an annual membership in January, you will be billed for January-December. If you decide to end membership in February, you will be refunded for the months of March through December.
You will be charged a re-enrollment fee of $300.00 to reestablish membership unless there are reasonable circumstances for canceling. We reserve the right to determine what is reasonable on a case-by-case basis. The purpose of the re-enrollment fee is to discourage abuse of our services.
